This survey has been designed by educational researchers at Johns Hopkins University to assess elements of school culture in your school.  

All responses are anonymous and will be analyzed by researchers at Johns Hopkins University.  School personnel will not have access to individual responses. Administrators should note, however, that their responses may be attributable in the case of small administrative teams.  If you agree to participate, you will be asked to answer questions for about 10 minutes. 

Participation in the survey is completely voluntary, and you may change your mind and stop at any time. There are no penalties for not participating or withdrawing from the survey.

*Parents and guardians:
This survey is for parents/guardians of K-12 students only (not Pre-K). Please complete one survey per household. Separated parents may complete one per parent. 

*School staff/parents:
If you are employed by the school and a parent at this school, please complete the survey once as a staff member AND once as a parent. 

*Teachers/administrators:
If you have both teaching and administrative duties, please complete the survey once as a teacher AND once as an administrator.
